Our mobile vaccination services will be offering 1st doses, 2nd doses, 3rd doses (for immunocompromised) and boosters. During the vaccine clinic, the vaccine team is federally required to collect the following vaccine recipient information: name, date of birth, gender, race, ethnicity, and phone number. Although not required, they do ask for mailing address also.
